What to bring for Your Hospital Stay:

  • Picture ID
  • All insurance information
  • Required Referral Letter from your Primary Care Physician (community care referral)
  • Any required co-payments/deposits (University Health Conway accepts VISA and MasterCard)
  • Current proof of income (for patients requesting free care classification)
  • Pajamas or nightgown (hospital provides patient gowns)
  • Dressing gown, slippers, underwear
  • Toiletries such as a comb, hairbrush, toothbrush and toothpaste, soap, deodorant, shampoo, a shaving kit, etc.
  • All personal articles should be labeled with your name.

Payment of Services:

  • If you are uninsured and unable to provide proof of Louisiana Residency at the time of registration, you may be required to pay a minimum $100.00 non-Louisiana Resident fee at each clinic visit
  • If your insurance plan does not list University Health Conway as an in-network provider, you may be responsible for up to 100% of the hospital and physician bills.

Optional Items:

Battery-operated equipment, such as a razor, lam, or radio (with earphones) is allowed.  Electric hair dryers (less than 800 watts) or curling irons are permitted but you should not use them while in bed or alone in the room.  You may bring a small amount of money for personal needs such as newspapers, snacks, etc.

For safety reasons, please leave the following items at home:

  • Butane or other gas-powered equipment
  • Electrical appliances
  • Jewelry
  • Appliances such as coffee maker, fan, humidifier, electric hairdryer (over 800 watts)
  • Large sums of money, credit cards, or valuable papers

Medication:

  • Medications - prescription bottles and over the counter medications
  • If your physician asks you to bring the medications you are currently taking (prescription, over the counter, herbal products) give them to the nurse or physician who admits you
  • A relative or friend may then return your medication to your home
  • If this is not possible, the medication will be kept for you until you leave the hospital
  • Remember to ask for your medication when you are informed of your discharge
  • Unless directed by your physician, pharmacist, or nurse, do not take any of your own medication while in the hospital

    Dentures:

    • If you wear dentures and remove them at night or have to remove them for surgery, please ensure that a family member keeps them for you and don't leave them in your hospital room.  Dentures can be picked up accidentally and thrown out when a room is being cleaned. Bring a container if you have one.
